- cost agent deregistered the wrong AMIs - its age filter ignored the "do-not-delete" tag the platform team uses
- the agent said "not reachable" but the vulnerable code path was behind a feature flag it never evaluated
- agent suggested indexing a low-cardinality boolean column and the planner ignored it -- the seq scan was already correct
