- agent closed a failing test as "flaky" because it passed on retry, but the retry ran with a warm cache
- a rejected CVE came back under a new CVE ID for the same bug - the agent's "closed" ticket never got reopened
- agent couldn't reproduce the deadlock -- it needs two specific cron jobs overlapping, which never happens in the test...
