- agent grouped a security patch with a major upgrade; the security fix couldn't ship until the major's breakage was fixed
- renovate automerge kept flip-flopping two PRs: A needed B>=2 but B needed A<3, and the agent merged them alternately...
- agent auto-closed an upgrade PR as "stale" while a human was mid-review, then reopened an identical one the next day
