Azure AD provisioning service failed: "resource operation completed with terminal provisioning state failed" error

Export

Fixes the Azure AD provisioning terminal state failed error by reading the failing step detail and repairing the target operation it names. Use when a provisioning cycle ends in terminal failure. Not for transient retries that succeed on the next cycle.

TL;DR

Open the failing provisioning log entry and fix the specific step it names instead of restarting blindly. The terminal state message is only a wrapper: the detail underneath points to a create, update, or delete that the target system rejected, such as a schema, permission, or uniqueness problem. Repair that cause in the mapping or the target, provision the user on demand to prove it, then restart the job.

resource operation completed with terminal provisioning state failed

Use this when

  • A provisioning cycle or on-demand run ends with the terminal provisioning state failed message.
  • The job history shows repeated failures for the same users across multiple cycles.
  • Other apps provision normally, so the tenant and network path are healthy.

Not for this skill when

  • A single cycle failed and the next automatic cycle succeeded. That was transient and needs no repair.
  • The error appears during initial tenant setup before any successful sync. Follow first-time configuration instead.

Steps

  1. In Microsoft Entra admin center, open the enterprise app, go to Provisioning, then Provisioning logs, and filter to failures in the affected window. Open the failed entry and expand every step, export, import, match, and action, and copy the inner error detail from the step that actually failed rather than quoting the outer terminal message. Verify: you have the inner code and description for the failing operation
  2. Map the inner detail to its class: schema or attribute errors point to the mapping page, permission errors point to the target service account rights, and uniqueness errors point to an existing target object. Check the app Provisioning page status banner at the same time for quarantine or credential warnings that change the diagnosis. Verify: the failure is assigned to one class with evidence from the log
  3. Fix the named cause at its source. For a schema failure, correct the attribute mapping or add the missing target attribute in the app schema. For a permission failure, grant the provisioning account the create and update rights in the target and retest the connection. For a duplicate, link the existing object as described in the matching settings. Verify: the corrected setting is saved and the test connection succeeds
  4. Run Provision on demand for one affected user and step through the fresh log entry to confirm export, match, and action all succeed. If on-demand still fails, read the new inner detail before changing anything else, because the terminal wrapper will look identical even when the cause moved. Verify: on-demand provisioning completes with a success status for that user
  5. Restart provisioning from the app Provisioning page so a clean full cycle runs with the repaired configuration, then monitor the next two cycles in the logs for the same users. Confirm the job status returns to healthy and the failure count stops growing. Verify: two consecutive cycles complete with no terminal failure for the previously affected users
  6. Record the inner error and its fix in the team runbook under the wrapper message wording, so the next engineer searches the phrase they actually see and lands on the step-level diagnosis. Verify: the runbook entry pairs the wrapper text with the inner codes seen in this incident

Why it happens

Entra reports the overall provisioning operation outcome with a generic terminal state, while the actionable rejection comes from the target system at step level. A SCIM service, directory, or SaaS API returns its own error, Entra marks the operation as terminally failed because retrying the identical operation cannot succeed, and the summary surfaces the wrapper instead of the cause. That design is correct: schema, permission, and duplicate errors need a configuration or record change, not retries. Reading the step detail converts an opaque failure into a single repairable cause.

Variants and edge cases

Terminal failure on delete operations only

When creates and updates succeed but deletes fail, the target usually protects the object, for example a primary admin or a licensed owner. Transfer ownership or unprotect the record in the target, then re-run the delete on demand.

Terminal failure after a mapping change

A newly required target attribute with no source value fails every create after the change. Roll back the requirement or supply a valid default expression, then retest one user before restarting the job.

Failure appears only in the full cycle, not on demand

On-demand runs bypass some scoping and delta state. Clear the job watermark only as a last resort after one user passes, and expect a long full reconciliation cycle to follow.

Compatibility: Applies to Microsoft Entra ID and legacy Azure AD provisioning for gallery, SCIM, and on-premises app provisioning jobs. Requires admin access to provisioning logs and mappings.
