## TL;DR
Repeated escalation of a low-risk account means the agent re-derives the same conclusion every run with no memory of having acted, often because a stale signal (an old ticket, a static field) keeps triggering it. Give the agent escalation state: what was escalated, when, and what changed since; require new evidence before re-escalating, and audit why that account's trigger never clears.

```text
agent loop detected: repeatedly escalating the same low-risk account, how to fix
```

## Use this when
- The same account is escalated on consecutive runs with no new events
- CSMs are ignoring the agent's escalations because of the noise
- The triggering signal appears static or stale

## Not for this skill when
- The account genuinely re-triggers on fresh deterioration each time
- Multiple different accounts loop, suggesting a global state bug

## Steps
1. List the account's escalations with the evidence each one cited. Verify: the repeated evidence is shown to be identical or stale
2. Trace the triggering signal to its source and check its freshness timestamp. Verify: the stale or non-clearing signal is identified
3. Add escalation state: last escalation time, cited evidence hash, and a cooldown window. Verify: a test run with unchanged evidence produces no new escalation
4. Require materially new evidence (new ticket, fresh usage drop, new sponsor signal) to escalate inside the cooldown. Verify: genuine new deterioration still escalates immediately
5. Clear or fix the stale signal at its source and review other accounts it may affect. Verify: the trigger population is audited, not just this account

Compatibility: Compatibility: agent-driven CSM alerting and playbook systems with run-to-run state stores, current versions.

## Why it happens
Stateless agents cannot learn that they already acted, so any persistent trigger becomes a loop. Escalation memory plus a new-evidence rule restores proportionality: the first escalation is the signal, repeats without new evidence are noise the system itself should suppress.

## Edge cases / pitfalls
- Cooldowns must never suppress a severity jump. A low-risk repeat can wait; a critical new event cannot.
- Evidence hashing should cover the underlying records, not the agent's summary text, which varies run to run.
- Audit ignored escalations portfolio-wide; alert fatigue from loops quietly disables human attention everywhere.
