## TL;DR

Log the outcome of every attempt, not just the final one, and alert on any test whose first attempt fails consistently. A test that never passes on attempt one is broken, not flaky, and retries are just hiding it. Make first-attempt failures visible in the report and fail the build when they cross a threshold.

## Error

```text
test_checkout_total ... ok (attempt 4 of 4)
# attempts 1-3 failed identically; the "pass" is an artifact of retries
```

## Steps

1. Turn on per-attempt logging in the runner (pytest: verbose output with a retry plugin that logs each attempt; jest: `--verbose` with retryTimes; Playwright: a reporter that records retry counts). Expected: the log shows attempt 1, 2, 3 outcomes separately, not just the final verdict.
2. After the run, list every test that failed on attempt 1, even if it passed later. Expected: you get a concrete list like "7 tests failed first attempt, 0 failed last attempt".
3. For each test on that list, check whether it EVER passes on attempt 1 across recent runs. Expected: tests that pass on attempt 1 sometimes are flaky; tests that never do are consistently broken.
4. Treat consistent first-attempt failures as failures: quarantine the test or file a bug, do not let retries keep it green. Expected: the test is marked failed or quarantined in the tracker with its first-attempt failure log attached.
5. Add a CI gate: fail the build if more than N tests (start with 0) fail on first attempt. Expected: the next run with a masked failure goes red instead of silently green.

## When to use

- The suite retries failed tests and the dashboard is green but failures keep "coming back".
- An agent added retries to make a red suite green without fixing anything.
- You see "attempt 3 of 3" style notes next to passing tests.

## When not to use

- The suite has no retry logic; every failure is already a first-attempt failure.
- The test is genuinely flaky (passes on attempt 1 most of the time); retries are the right tool there.
- You are still diagnosing; gather per-attempt data first, then decide.

## Tool compatibility

- pytest with pytest-rerunfailures or flaky plugins, jest/vitest with retryTimes, Playwright with retries in config.
- Any CI that can parse per-attempt logs (GitHub Actions, GitLab CI, Jenkins).

## Variant phrasings

### Retry loop hiding failing tests

If the only green comes after retries, the test is red. Per-attempt logging makes the hiding impossible.

### Test passes only on retry

Distinguish "sometimes passes first try" (flaky, keep retrying) from "never passes first try" (broken, fix it).

### Detect masked test failures

The metric is first-attempt pass rate per test; track it over runs and alert on drops.

## Why it happens

Retries were built for flakes: transient failures that clear on their own. But a retry loop cannot tell a flake from a real failure, so a test that fails deterministically on attempt 1 gets the same treatment as a network blip. Each green-after-retry trains everyone to ignore the underlying breakage.

## Edge cases

- Order-dependent tests fail on attempt 1 only in the full suite; run them alone to check whether the failure is real or ordering.
- Retry plugins sometimes count setup failures as attempt failures; separate setup errors from test-body failures in the log.
- Lowering the retry count to expose failures is tempting but noisy; keep retries and add the first-attempt gate instead.
- An agent that owns the retry config may raise the retry count when the gate fires; review retry-config changes like code changes.
