Immutable solution version

shared account review failed: password vault checkout audit log missing error

Version 1

Complete solution

Fixes a shared account review that fails because a password vault checkout audit log is missing entries by tracing checkout events across vault, application and log pipeline. Use when shared account use cannot be evidenced. Not for individual named accounts, which should not be shared at all.

TL;DR

Rebuild the evidence trail from the systems around the vault before concluding checkouts went unlogged, because the events usually exist somewhere and the gap is in collection. Compare vault session records, target system sign-in logs and the central log store for one known checkout, and find where the chain breaks, commonly a log forwarder, a vault audit setting or a retention mismatch. Repair the collection for the affected window, document any truly unrecoverable gap honestly for the auditors, and add a monthly reconciliation so missing logs surface within weeks, not at audit time.

shared account review failed: password vault checkout audit log missing error

Use this when

  • The review cannot match shared account use to named individuals for a period.
  • Vault audit reports show gaps or far fewer checkouts than the team expects.
  • A SIEM or log archive query returns nothing for known checkout dates.

Not for this skill when

  • There is no shared account use in the period and the log is correctly empty. Record that confirmation instead of hunting for missing data.
  • A checkout happened outside the vault entirely. That is a policy breach investigation, not a logging repair.

Steps

  1. Take one checkout the team is certain happened, with the date, account and person, and search for it in the vault own audit or session history first. Verify: the known checkout is found or proven absent in the vault records.
  2. Search the target system sign-in or usage logs for the same account and window to see whether the use itself was recorded even if the vault link is missing. Verify: target side evidence for the known checkout is located or ruled out.
  3. Follow the log path into the central store or SIEM, checking forwarder status, index selection and retention for the vault source over the gap window. Verify: the break point in the chain is identified with dates.
  4. Check vault audit configuration for the affected safes or platforms, confirming checkout and use events are enabled at the level the review needs. Verify: audit settings are confirmed or corrected per safe.
  5. Repair the collection fault, backfill logs from the vault or target where the source still holds them, and mark any window that cannot be reconstructed as a declared evidence gap. Verify: recovered logs are in the store and unrecoverable windows are declared in writing.
  6. Reconcile counts for a recent month across vault sessions, target sign-ins and central logs, investigating any mismatch above a small agreed threshold. Verify: the three sources agree within the threshold for the test month.
  7. Add the reconciliation as a monthly control with an owner, and file this incident and its gap declaration in the review evidence. Verify: the monthly control exists and the incident is documented for auditors.

Why it happens

Shared accounts are tolerated only because the vault checkout log is supposed to restore individual accountability. When that log has holes, the control fails even if the underlying behaviour was innocent, which is why auditors treat missing logs seriously. In practice the events are usually generated and lost downstream, in forwarding, indexing or retention, so a single known checkout traced through each hop finds the break far faster than reviewing settings in the abstract. Honest gap declarations, paired with a working monthly reconciliation, are what rebuild auditor confidence.

Variants and edge cases

cannot prove who used shared admin account last quarter

The accountability phrasing. Reconstruct from target logs where possible and declare the rest; never fabricate the trail.

CyberArk or vault audit log gap for checkout events

The vault phrasing. Check per safe audit settings before blaming the log pipeline; both are common.

SIEM missing vault logs for audit period

The pipeline phrasing. Retention mismatch is the classic cause; the vault may still hold the source records for backfill.

Edge cases to check before closing the review cycle:

  • Legal or regulator requests can require preserving even partial logs under hold. Coordinate before any log cleanup during repair.
  • Break glass accounts have their own evidence expectations. Keep their log checks separate and more frequent.
  • A forwarder fixed today does not backfill automatically. Always check whether the gap window needs an explicit backfill job.

Compatibility: Password vault auditing in CyberArk and similar vaults, SIEM log collection, shared and break glass account review evidence.
