Complete solution

Explains how to track action items from postmortems so they actually get done: one list, one owner per item, one due date, one weekly review. Use when postmortem follow-ups keep dying. Does not cover writing the postmortem.

TL;DR

Every action item gets an owner, a due date, and a single tracking list that gets reviewed weekly. Postmortem action items die in docs nobody reopens. The fix is mechanical: one list, one owner of the list, one weekly check until the list is empty.

Error / query

how to track action items from postmortems

The postmortems are written, the action items are in them, and nothing is getting done. You need tracking, not more docs.

Use this skill when

  • postmortem action items keep dying quietly
  • you need a single list of who owes what by when
  • leadership asks "did we actually fix it" and nobody knows
  • youre setting up the postmortem process for the first time

Not for this skill when

  • you need the postmortem template itself (a different skill)
  • you need the review meeting agenda (a different skill)
  • the items are feature work, not incident follow-ups (use the normal backlog)

Steps

1. Extract action items into one tracking file

Pull every open item out of the postmortem docs into a single list. Scattered items are invisible items.

grep -h "^\s*|" ./postmortems/*.md 2>/dev/null | grep -v "^\s*| *----" | head -30
echo "--- candidate action items above; consolidate the real ones below ---"

Expected: every action item visible in one place. If the grep finds nothing, the postmortems have no parseable items, which is itself the first thing to fix.

2. Give each item an owner and a due date in the tracker

One CSV, four columns. Items owned by "the team" get a name before they enter the list.

cat > /tmp/action-tracker.csv <<'EOF'
item,owner,due,status
fix retry backoff on payments-api,alex,2026-10-11,open
add alert for queue depth,priya,2026-10-18,open
document the failover procedure,sam,2026-10-25,open
EOF
cat /tmp/action-tracker.csv

Expected: a tracker where every row has a named owner, a date, and a status. No owner, no entry.

3. Review the tracker in the weekly ops meeting

Read the open items out loud every week. Social accountability is the whole mechanism.

awk -F, 'NR>1 && $4=="open" {print $2": "$1" (due "$3")"}' /tmp/action-tracker.csv

Expected: a spoken list of who owes what by when. Overdue items get named out loud and re-dated or escalated on the spot.

Variant phrasings

postmortem follow-up tracking

Same fix: the single tracker plus weekly review in steps 2 and 3 is the follow-up system. Nothing fancier needed.

how to make sure incident action items get done

Same fix: owners, dates, one list, weekly review. Items get done because someone is asked about them every week, not because they were written down.

incident action item tracker template

Same fix: the CSV in step 2 is the template. Four columns, no more.

Why it happens

Writing the item down felt like doing it, and the postmortem doc felt like the finish line. But docs dont get reopened, owners forget, and dates slip silently. A single list reviewed weekly works because it replaces good intentions with a recurring moment of accountability.

Edge cases and pitfalls

  • Items owned by "the team": assign a name. Shared ownership is no ownership.
  • Items that never get prioritized: they go into sprint planning, not a side list. A side list is a graveyard.
  • Tracker has 50 stale items: archive anything older than 90 days and re-add only what still matters. A bloated tracker gets ignored like a bloated inbox.
  • Cross-team items: the owning teams manager is the owner. Peer-to-peer ownership across teams drifts.
