Keycloak 26.3.0 (and 26.3.1) failed to start with Liquibase/database driver errors during migration, e.g. the changelog lock or driver initialization blowing up at boot. The regression was introduced in the 26.3.0 release's dependency set and hit users on upgrade regardless of their database. The failure happened at boot during migration, before Keycloak could serve any traffic.

This was a regression in Keycloak 26.3.0 and 26.3.1; upgrade to 26.3.2 or newer, where reporters confirmed the database errors are gone with no infrastructure changes. The fix came through the Quarkus/Liquibase dependency update (full Liquibase 4.33 support landed in 26.4).
